यसको सेवा Date of Enroll को ठिक ३१ दिन पछी मात्र उपभोग गर्न सकिन्छ , त्यस अगाडि भने २५% छुटमा सेवा उपभोग गर्नसकिन्छ।

Facility from farmacy can be taken as per the rules of the company.
फार्मेसी भने कम्पनिको नियम अनुसार उपभोग गर्न पाउनेछ।

यो योजना अन्तर्गत आजिवन नि:शुल्क स्वास्थ्य सेवा वा उपचार पाइनेछ।

if the unity members want to change the series from unity life to unity hospital, the overhaul service charge will be taken the rate of 30% from unity life. But the ovehauls will bea provided one month later from the date of transfer. I tacit all the mentioned rules and regulation for the health plan; I am highly interested and agree to do
यस योजनाबाट अरु नै योजनामा र अरु योजनाबाट यस योजनामा पनि टान्सफर गर्न सकिन्छ जसको लागि भने ३०% सेवा शुल्क लाग्नेछ। सेवा भने Date of Transfer को ठिक एक महिना पछि मात्र लागुहुनेछ ।

This plan can not be cancelled after first Wednesday(i.e. first Wednesday after the date of enroll)onwards.
यो योजना Date of Enroll को पहिलो बुधवार पछि रद्द गर्न सक्ने छैन ।

योजना परिवर्तन गर्न चाहने महानुभावहरुले निम्न कागजातहरु पेश गर्नु पर्नेछ।
To change the series, the individuals must have to submit following documents:
i.
Application with full signature of applicant and ID copy.(original/ photo copy)
हस्ताक्षर सहितको निवेदन र परिचय पत्रको नक्कल/सक्कल
ii.
Application of sponsor with full signature and ID copy.(original/ photo copy)
हस्ताक्षर सहित सिफारिसकर्ताको निवेदन र परिचय पत्रकको नक्कल/सक्कल
iii.
The service charge of the series is 30%.
सेवा शुल्क वापत ३०% रकम
